Appointments

• Our providers operate on an appointment basis. Your primary care provider may schedule an appointment during standard operating hours (Monday – Friday 8 to 5). If your condition requires immediate assistance, please dial 911 or go to the nearest emergency room.

• If you need to cancel or reschedule, please do so at least 24 hours prior to your appointment time.

• Arrive on time for your appointment. 

• New patients must bring completed paper work to appointment

• Bring your insurance cards and co-payment with you to your appointment

• For your protection, a photo ID will be taken and attached to your electronic record
