What do I need to bring with me to my appointment?

When coming to each appointment, please make sure to bring all current medications or a list of all current medications you are taking including any over the counter medications. You will also need to bring your most current insurance information and a photo ID. If this is your first visit please bring in your completed new patient paper work that you can find here: (Link to patient forms page). If this is a subsequent visit, please make sure to inform the front desk staff of any changes to your address, phone number and all other contact information.

How do I get a prescription refilled?

You can obtain a refill of any medications at the time of your visit. If you require medication refills in between visits, you can reach the nurses’ line at 434-372-0900 and follow the prompts. Please allow 48 for the completion of all refill requests.

What if I need assistance after hours?

After normal business hours, urgent calls are handled by the provider on call. To reach the provider on call, please contact VCU Health Community Memorial Hospital at 434-447-3151 and press option 0 for the operator. For emergencies please call 911 , or go to your nearest Emergency Room. Please keep these calls to emergent or urgent situations only, you should call our office during normal business hours for all non-urgent questions.

Can I pay my bills online?

Currently we are unable to process payments on line via this website, however this feature will be available soon through our patient portal. Although we are owned and operated by VCU Health CMH, please do not attempt to pay your bill for Clarksville Primary Care Center by using the VCU Health CMH website.
