Patients with Managed Care Insurance
Patients with Managed Care Insurance must have an authorization at the time of the appointment to receive care, including diagnostic testing and/or surgery.
It is the patient's responsibility to make sure an authorization is obtained before each visit. If patients come to our office without the appropriate authorization, they will have the option of rescheduling their appointments or paying up-front for the visit.
